The dosage and administration of encorafenib vary depending on the indication! Encorafenib is a prescription drug, and the dosage and treatment course must be determined by a medical oncologist or corresponding specialist. Do not adjust the dosage or discontinue the medication on your own.
Recommended Dosage
1. Melanoma (in combination with binimetinib)
Encorafenib 450 mg (six 75 mg capsules) orally once daily; binimetinib 45 mg twice daily. Continue until disease progression or unacceptable toxicity occurs.
2. Non-Small Cell Lung Cancer (in combination with binimetinib)
Encorafenib 450 mg (six capsules) orally once daily; binimetinib 45 mg twice daily. Continue until disease progression or unacceptable toxicity occurs.
3. Metastatic Colorectal Cancer (in combination with cetuximab)
Encorafenib 300 mg (four capsules) orally once daily. Continue until disease progression or unacceptable toxicity occurs.
4. First-Line Metastatic Colorectal Cancer (in combination with cetuximab + mFOLFOX6 or FOLFIRI)
Encorafenib is likewise 300 mg once daily.
Precautions: The maximum upper limit for encorafenib is 450 mg once daily.
Administration Guidelines
1. Method of Administration
Avoid grapefruit and grapefruit juice. Swallow whole with water. May be taken with or without food. Once daily, and it is recommended to take it at approximately the same time every day.
2. Missed Dose
Only make up for a missed dose if it is more than 12 hours away from the next scheduled dose, taking it at the original dose. If it is less than 12 hours until the next dose, skip that dose and take the next one according to the original schedule. Never take a double dose to make up for a missed one.
3. Vomiting After Administration
Do not take an extra dose; take the next dose at the scheduled time.
Dose Adjustments for Adverse Reactions
1. Melanoma and Non-Small Cell Lung Cancer
First level reduction: Reduce to 300 mg (four capsules) once daily.
Second level reduction: Reduce to 225 mg (three capsules) once daily.
If 225 mg once daily is still not tolerated, permanently discontinue.
2. Metastatic Colorectal Cancer
First level reduction: Reduce to 225 mg (three capsules) once daily.
Second level reduction: Reduce to 150 mg (two capsules) once daily.
If 150 mg once daily is still not tolerated, permanently discontinue.
Changes in Combination Therapy Status
1. If binimetinib is temporarily interrupted or permanently discontinued, encorafenib should be reduced to a maximum of 300 mg (four capsules) daily until binimetinib therapy is resumed.
2. If encorafenib is used in combination with cetuximab and chemotherapy: When cetuximab is permanently discontinued, encorafenib may be continued with chemotherapy; when both cetuximab and chemotherapy are permanently discontinued simultaneously, encorafenib should also be permanently discontinued.
3. If encorafenib is used solely in combination with cetuximab: When cetuximab is permanently discontinued, encorafenib must also be permanently discontinued.
Dose Adjustments for Strong or Moderate CYP3A4 Inhibitors
Avoid co-administration whenever possible. If co-administration cannot be avoided, reduce the dose level according to the current daily dose.
1. Current daily dose of 450 mg: Reduce to 225 mg (three capsules) when combined with a moderate CYP3A4 inhibitor; reduce to 150 mg (two capsules) when combined with a strong inhibitor.
2. Current daily dose of 300 mg: Reduce to 150 mg (two capsules) when combined with a moderate inhibitor; reduce to 75 mg (one capsule) when combined with a strong inhibitor.
3. Current daily dose of 225 mg: Reduce to 75 mg (one capsule) when combined with a moderate inhibitor; reduce to 75 mg (one capsule) when combined with a strong inhibitor.
4. Current daily dose of 150 mg: Reduce to 75 mg (one capsule) when combined with either a moderate or strong inhibitor.
After discontinuing the CYP3A4 inhibitor, wait for 3 to 5 elimination half-lives before resuming the encorafenib dose used prior to starting the inhibitor.
